Your privacy is very important to us. Although we require certain information from you to allow us to provide particular services, our policy is to otherwise permit you to choose how much information you provide to us.
I. Collection of Information
The PT-Helper® App (“App”) and PT-Helper CONNECT (“CONNECT”, together hereinafter sometimes referred to as the “Applications”) collect three types of personal information: (i) information that you voluntarily provide to us, such as registration information, and (ii) information that is derived from your use of the Applications, such as technical information, and (iii) personal health information provided by users of the Applications to utilize services provided by the Applications.
a. Voluntary Registration Information. In order to fully access the Applications, you will be required to complete a registration process to establish a username and password, during which we will collect personal information about you. The information may include your name, address, and email address. We may also collect and store personally identifiable information (username, name, contact information) of Users.
b. Technical Information. We may also collect technical information, which cannot be matched or used to identify a specific individual, such as browser type, language preference and the date and time of each use. Our collection of technical information allows us to track usage of the App, evaluate trends and market the App. Furthermore, we may collect device specific information (such as your hardware model, operating system version, unique device identifiers and mobile network information). We use the information we collect from all of our services to provide, maintain, protect and improve them, to develop new services and to protect our users.
c. Personal Health Information. In utilizing certain features and services provided by the Applications, you may provide certain information or content containing personal health information. This may include information and data related to the provision of health care to patients, their health status, completion of certain treatments, regimens, exercises, and other tasks, medical records, and treatments performed by Healthcare providers. When providing services to Healthcare Providers through use of the Applications, we may receive or create communications containing Personal Health Information related to users, like you, who are served by these Healthcare Providers. We will only use and disclose this Personal Health Information as necessary to perform our services for those Healthcare Providers, and only as permitted or required by law.
II. Third Party Vendors. You may be requested to submit personal information such as your name, phone number, email address and credit card information to third party vendors of PT Helper in order to purchase the App.
Disclosure of Information
PT Helper may access or disclose voluntary registration and personally identifiable information we collect about you: (1) to those employees, contractors or third party vendors of PT Helper that need to have such information in order for you to use the App; (2) to comply with valid legal requirements such as law, regulation, warrant, subpoena or court order; (3) to protect the rights or property of PT Helper or our customers, including the enforcement of our agreements or policies governing your use of the services; or (4) to act on a good faith belief that such access or disclosure is necessary to protect the personal safety of PT Helpers officers, employees, agents or customers.
PT Helper may access or disclose voluntary personal health information in order for the user to utilize certain services provided in the Applications. PT Helper only uses your personal information for the original purpose it was given. Your personal information will not be sold or otherwise transferred to unaffiliated third parties without your approval at the time of collection.
We take the security of your data seriously. We use commercially reasonable efforts including a variety of technologies to prevent unauthorized access, use or disclosure and we store information on computer systems with limited access. At the same time, we urge you to take every precaution to protect your personal information when you use the Applications or connect to the Internet. We cannot guarantee the absolute security of your personal information, nor can we guarantee that the information that you provide will not be intercepted while being transmitted over the Internet.
